Provider Solutions & Development Palliative Care Social Worker in Seattle, Washington
Edmonds, Washington -- Swedish Medical Group is seeking an excellent Palliative Care Social Worker to work collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness. The Palliative Care Social Worker is guided by the NASW Code of Ethics and the Swedish Mission and Vision in daily practice. Full-time position (1.0 FTE); at least one year of experience required.
Responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, facilitation of family meetings and more.
Will assume a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes.
